Keep in mind that we don’t have primary doctors or internist.
The process is simple:
- Download our referrals form, fill it out and send it via fax or email with the patient’s information to 661.327.7255 or referrals@csfsurgery.com
*Please make sure to attach the patient’s medical diagnosis, exam results, images, and laboratories.
- Our highly trained bilingual patient advocates handle each case from start to finish and will fax or e-mail a case update to the referring facility.
With CSF Surgery the referral process is direct!
